The film's plot begins in 1854, when a wounded soldier named Puang encounters a ghostly woman in black who offers him immortality in exchange for serving as her host. The ghost, known as Puang's ghoul, controls him for over a century. In 1975, three years after the events of the first film, Yak has become a hunter of evil beings, seeking vengeance for his sister Yam. He is joined by his friend Sarge and five others: Luechai, his nephews Jirasak and Nawin, necromancer Sem, and porter Khom. They journey into Dong Khomot forest, where they encounter deadly supernatural beings. Sem is killed by a king cobra, Jirasak accidentally kills Khom and then himself, and Nawin is devoured by a werecat. Only Yak, Sarge, and Luechai survive to find Puang, who reveals he has hidden from the ghoul using an amulet. After Luechai cuts off Puang's arm, the ghoul appears, kills Luechai and Puang, and forces Yak to shoot himself. Yak survives due to a protective yantra on a voodoo doll. The ghoul then attacks Yak's family at a hotel, possessing Boonyen and forcing her to chase Yee with a pickaxe. Yak and Sarge return, and with Yad's help, they cut off the ghoul's tongue and destroy her, freeing all her victims.

Death Whisperer 2 is significant as a major commercial success in Thai cinema, earning ฿80 million on its first day—the highest first-day gross for a Thai film in history. After seven days, it grossed ฿450 million in Thailand, and after nine days, over ฿550 million nationwide. It became the highest-grossing domestic film of 2024 and the fourth-highest grossing domestic film of all time. In Vietnam, it set a record for highest advance booking with over 30,000 tickets sold and grossed ฿13 million on its first day, the highest for any Thai film there. The film was shot entirely for IMAX release, like its predecessor. It is inspired by Sam Raimi's Evil Dead franchise, but differs from the first film by incorporating many action scenes, making it more of an action horror film. This shift came from lead actor Nadech Kugimiya during script discussions. The hotel chase scene was inspired by The Shining. The film's production included a prayer ceremony on February 15, 2024, and a press conference on February 28, 2024. A gala premiere was held on October 8, 2024, at Suralai Hall, Iconsiam, two days before the first screening.
